General guidance on the safe use of ionising radiation and forms that should be completed for incidents or equipment/controlled area handover.

The RPA will be able to determine whether the incident must be reported to a government agency (e.g. CQC), and liaise as required with the agencies. Where the incident involves the medical radiation exposure of a patient, it would be helpful if the information detailed on the form linked above were provided to the RPA.

For all X-ray equipment servicing, it is recommended that this form, produced by AXEM & SRP, is completed by both a representative of the radiation Employer and the service Engineer/Apps Specialist.
