Medical Radiation Exposures Incident - Internal Form for RPA
Summary:
The RPA will be able to determine whether the incident must be reported to a government agency (e.g. HSE, CQC, EA, DfT, etc.), and liaise as required with the agencies. Where the incident involves the medical radiation exposure of a patient, it would be helpful if the information detailed on the form linked above were provided to the RPA.
